Thank you for your interest in referring a client to Anchored Hope Wellness Center. We value collaboration with providers, schools, and community organizations and are committed to providing high-quality mental health care.

To protect client privacy and comply with confidentiality laws, referral forms must be submitted using secure methods. Please do not email referral forms that include personal or clinical information. Completed forms may be faxed to 1-508-455-1099.

Once a referral is received, our team will confirm receipt and, with appropriate consent, contact the client or family to discuss services and scheduling.
